1. Formal Training Centers
   1. Must execute a formal agreement between CARE and the hospital/ health system/ group.
   2. The executed Accredited Center agreement gives the center access to all of the training materials.
   3. Training is focused on internal staff only. Accredited Centers cannot open up training to outside staff.
   4. To maintain center status, the center must:
      1. Complete and pass periodic audits
      2. Trainers must maintain certification status
      3. Notify CARE at least 30 days in advance of planned courses
         1. To facilitate online registration.
         2. To gain access to E-learning pre-course materials.
         3. To permit the shipping of course books in advance of your course.
   5. Trainers are invited to participate in the international accreditation process.
   6. There are no fees to maintain center status.
   7. Accredited centers may transition to Centers of Excellence upon request:
      1. An updated agreement will be required.
      2. There is no fee to make this change.
   8. A current agreement is required to offer courses to staff.
   9. Must successfully complete biennial audits beginning at the end of the first year of training
2. Centers of Excellence (COE)
   1. For centers interested in a higher level of recognition.
   2. The COE agreement gives the center access to all of the training materials including periodic updates.
   3. Training is focused initially on internal staff only.
      1. Upon successful completion of the first audit, the center may open up courses to outside staff.
      2. Eligibility is determined by audit performance.
      3. Remediation may be required prior to opening courses to outside staff.
      4. On successful completion of first audit, a CSU-ALS COE plaque will be awarded for display in your ICU.
   4. To maintain COE center status, the center must:
      1. Complete and pass periodic audits.
      2. Trainers must maintain certification status.
      3. Notify CARE at least 30 days in advance of planned courses:
         1. To facilitate online registration.
         2. To gain access to E-learning pre-course materials.
         3. To permit the shipping of course books in advance of your course.
      4. Keep 90% of staff trained at all times (COE determines who these staff should be)
   5. COE Trainers
      1. Are invited to participate in the international accreditation process.
      2. Are eligible to be invited to travel and teach courses nationally on an as needed basis
   6. COEs are invited to participate in national/ international research initiatives
   7. There are no fees to maintain center status
   8. A current agreement is required to offer courses to staff
   9. Must successfully complete biennial audits beginning at the end of the first year of training
